To get a health card in Alberta, you have to fill out the Alberta Health Care Insurance Plan (AHCIP) Coverage Application Form, and you must provide some supporting documents to prove your Alberta residency, identity, as well as your legal entitlement to be in Canada.
You can apply for a health card in person at a registry agent office or by mail.
To register for Alberta Health Care Insurance Plan coverage:
- Get an AHCIP Coverage Application form online or from an authorized registry agent office
- Fill it out
- Collect the required documents
- Take your application and original documents to a Registry Agent Office, or mail your application and photocopies (front and back as applicable) of your documents to Alberta Health
Note: According to Alberta Health, when you apply for a health card in person at a registry agent office, the processing time can take up to 5 days. When you register by mail, the processing time can take up to 6 weeks.
